Overview:

The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families & caregivers.

Essential Job Functions

  • Coordinates the assessment and delivery of grief counseling needs and services.
  • Plans for Bereavement Program development, expansion and refinement through annual evaluation of services.
  • Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling.
  • Assists as needed in evaluating and planning services to meet hospice personnel support needs.
  • Participates in hospice activities such as in service education, hospice personnel meetings, and relevant committees.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Determines the level of care and support of bereaving families and significant others following the death of a hospice patient.
  • Reflects the needs of the family in the bereavement care plan and updates and revisions are conducted on an ongoing basis.
  • Implements the Plan of Care provisions related to the bereavement needs of families & significant others.

Qualifications:

  • Bachelor’s degree or equivalent theological degree from college or university OR,
  • Master’s degree in Social Work with qualifications of experienced clinical grief counseling.
  • Experience with families/caregivers and patients through death & dying issues.
  • Ability to work as a team member.
  • Knowledge and commitment to hospice philosophy.
  • Good verbal and written communication, and organization skills.
  • Understanding of grief response and experience with counseling.
  • Licensed driver with an automobile that is insured.
