Assess and provide treatment to patients in their homes according to physician orders. Oversee medication compliance and evaluate side effects. Provide education to patients and caregivers regarding diagnosis, medications, and care of patients. Work with physician to prevent re-hospitalization of patients. Supervise LVN.

Triage, admit, and discharge patients per protocol. Assess and monitor patients' physical and mental health and document in electronic health record. Provide medication and education on usage and side effects. Delegate appropriate tasks to behavioral health technicians and provide supervision as needed. Monitor patient and staff safety continually.

Conduct annual physical assessments of clients, delegate medication administration to staff, educate staff on medication administration, provide in-services on new medications and any changes in medications or doses. Document clients' medical appointments, update and distribute Medication Administration Records (MARs) to group home. Be a resource and available to staff at all times.
